The wound management service supports people with surgical, traumatic or chronic wounds living in the community or in residential care. Our nurses are skilled in wound care and we have a Wound Nurse Specialist and Nurse Practitioner to support best practice and ensure the best outcome for clients.

HealthCare NZ nurses support people with end stage or terminal illness assisting them to die with dignity, with the support of their family/whānau at home. Among the services provided, clients are supported with pain management and other symptom relief, and emotional support.

We provide Peripherally Inserted Central Catheter (PICC) line and intravenous therapy support to enable clients requiring antibiotics or chemotherapy to live at home. Our nurses undergo specific training and competency certification to provide this service.

Clients with diabetes are supported to live in the community through a number of nursing services. HealthCare NZ nurses support clients to manage their diabetes independently by monitoring blood glucose levels and assisting with, or administering, tablets or insulin.
Nurses work closely with the client to help them manage their condition. They educate them about their diabetes and work with their Primary Provider to ensure the best possible outcomes.

To support people living in the community with chronic pulmonary or respiratory disease, we provide chronic disease management. Our nurses have specialised competencies to support people with chronic conditions to be independent. Nurses assess and monitor these clients to ensure they receive timely treatment, advice and management to help them stay at home.

We provide post discharge surgical services to people who have undergone cardiothoracic surgery. This service enables people to be discharged home early where they are provided with assessment, monitoring, wound management and education to support their initial recovery. Our nurses have specialist competencies to support the needs of these clients, helping to prevent complications or readmission to secondary services.

People requiring continence management at home are supported with nurses experienced in continence care. Nurses work with other primary and specialist providers to assist clients to manage at home and be supported with catheterisation techniques or devices.

We have a number of complex care teams that provide clinical oversight and support to people rehabilitating with serious injuries. Our Support Workers are educated, trained and supervised by nurses to provide complex care to clients. These teams work closely with other providers and agencies to provide social support with a restorative focus.

ACC contracts HealthCare NZ to assess claimants in relation to their rehabilitation needs following an accident.
We
have a team of highly qualified and experienced assessors who have been
specifically approved by ACC to assess: Nursing requirements (wound
care, continence care, complex care, and supplies); household
management; personal care (including complex care); and equipment needs.
Our nurses support people to mange their medications at home. They ensure clients receive their medication in a safe and timely manner that complies with current and relevant legislated and regulatory requirements. This support prompts and assists the client to take their medication or helps them to administer the medication themselves.

Definition of medically fragile:
A medically fragile child has a medical condition which is under the care of a paediatric specialist. They will require daily nursing care and/or have significant risk of a life-threatening event in the next year.
The medically fragile children service aims to promote quality of family life, community participation and maximum independence for children from birth to 14 years of age.
It offers short-term respite support that meets the needs of the child and their family/whānau. The focus of the service is to support the family/whānau to continue to care for their child. The service is developed in consultation with the family/whānau, who are also involved in planning and implementing the support.
The service is delivered in the home or other community setting by skilled and experienced staff who are overseen by a Registered Nurse. Services may include:

You will be referred to the medically fragile children service through a needs assessment and service coordination team (NASC). For a first referral, children must be referred to the NASC by the paediatric service at the hospital.
